SPONSORSHIP

This form is a letter for sponsorship. If you have a company or business that would like to sponsor your player, hand out a letter to them - Have them send the form and check to the address above. Also, have them attach the logo.

20/20

This form is a letter for sponsorship from friends and family. The letter is to ask $20 from 20 people, and that can be put against your account.
